Situation
The integrated and decentralised Hospital at Home (HaH) form of care closes a gap between the inpatient and outpatient setting. Patients receive the medical and nursing care they need in their familiar surroundings. To date, there is a lack of scientific evidence in Switzerland on the extent to which this form of care can be implemented safely, effectively, cost-effectively and to the satisfaction of all parties involved.
Course of action
A retrospective, descriptive analysis of all patients treated in HaH of the Arlesheim Hospital between January and December 2024 will be conducted. The safety and effectiveness of care will be analysed using longitudinal data on the patient's state of health, complications and mortality, and the satisfaction of patients and relatives will be recorded using questionnaires and a telephone recall 30 days after discharge. The daily presentation of interdisciplinary and nursing rounds is used to determine the amount of nursing care required. In order to gain insights into cost-effectiveness, the care situation before and after the HaH-stay will be depicted and rehospitalisations will be assessed.
